Privacy statement

Helen Talaga takes the collection and storage of your information seriously and complies with the General Data Protection Regulations 2018. Helen Talaga acts as the Data Controller and is responsible for maintaining all records as a Sole Trader.

Information collected about you and how it is used

At or just before your first appointment I will ask you to complete a ‘patient information and consent form’; this will allow me to record your contact details (e.g. name, address, contact number, GP) and any relevant medical history. During the physiotherapy sessions, I will take notes which allow me to comply with my legal note taking obligations. Notes will not be shared with anyone unless authorised by the patient. No automated decisions are made based on the data that you provide.


Paper notes

Any information written down on paper is transferred onto an online, password protected, secure system to which only Helen Talaga has access. Once transferred, the paper notes are shredded, unless you request their return. Any Information captured using an online form is also deleted once transferred. 

Email and telephone number

If you have emailed Helen Talaga or have consented to Helen emailing you, then your email address will be stored in an account to which only Helen has access. Helen will store your contact phone number on her phone which is password protected.  

Data retention

At the end of your physiotherapy treatment, Helen Talaga will hold your details and notes electronically for a period of 7 years. If you are under the age of 18 years old, records will be kept for 7 years after you reach the age of 18 years old. After this date, all data will be securely deleted and will not be recoverable by anyone, including Helen Talaga.

Your rights regarding the data held about you

  • You have the right of access, to see a copy of the data held against you by Helen Talaga.
  • You have the right to rectification, should you believe that any information held on you is incorrect then you can ask that it is corrected
  • You have the right to erasure; you can request that your data is deleted or destroyed
  • You have the right to restrict processing, being able to limit the amount of data being used
  • You have the right of data portability, allowing you to request that information is moved should you want to work with a different therapist.
  • You have the right to object, allowing you to request that Helen stops using your data.
